Position Details


Position: Home Health RN / Case Manager
Facility: VNA Hospice of the Southwest Region
Shift: 8:00 AM – 4:00 PM
Schedule: Tuesday–Saturday or Sunday–Thursday
Guaranteed Hours: 40 hours/week
Contract: 13 weeks
 

 

Required Experience

  • OASIS experience is required
  • Home Health experience
  • Case management experience
  • Experience developing and managing patient plans of care
  • Experience with high-tech nursing skills preferred/expected, including:
    • IV use and maintenance
    • Wound VAC
    • Complex wound care
  • Experience with McKesson processes
  • Strong documentation and regulatory compliance experience

Key Responsibilities

  • Perform comprehensive patient and caregiver assessments.
  • Develop and communicate individualized plans of care.
  • Serve as the Case Manager for assigned patients.
  • Complete accurate and timely OASIS documentation.
  • Provide skilled nursing interventions and medical treatments.
  • Coordinate care with physicians, providers, PT, social work, vendors, and other disciplines.
  • Supervise and collaborate with LPNs within the applicable scope of practice.
  • Provide patient/caregiver education.
  • Coordinate admissions, discharges, and transfers.
  • Conduct case conferences and participate in peer-review audits.
  • Monitor and evaluate patient responses to the plan of care.
  • Maintain accurate clinical documentation.
  • Coordinate HCA services and supervise care as required.
  • Advocate for patient and family needs.
