Position Details
Position: Home Health RN / Case Manager
Facility: VNA Hospice of the Southwest Region
Shift: 8:00 AM – 4:00 PM
Schedule: Tuesday–Saturday or Sunday–Thursday
Guaranteed Hours: 40 hours/week
Contract: 13 weeks
Required Experience
- OASIS experience is required
- Home Health experience
- Case management experience
- Experience developing and managing patient plans of care
- Experience with high-tech nursing skills preferred/expected, including:
- IV use and maintenance
- Wound VAC
- Complex wound care
- Experience with McKesson processes
- Strong documentation and regulatory compliance experience
Key Responsibilities
- Perform comprehensive patient and caregiver assessments.
- Develop and communicate individualized plans of care.
- Serve as the Case Manager for assigned patients.
- Complete accurate and timely OASIS documentation.
- Provide skilled nursing interventions and medical treatments.
- Coordinate care with physicians, providers, PT, social work, vendors, and other disciplines.
- Supervise and collaborate with LPNs within the applicable scope of practice.
- Provide patient/caregiver education.
- Coordinate admissions, discharges, and transfers.
- Conduct case conferences and participate in peer-review audits.
- Monitor and evaluate patient responses to the plan of care.
- Maintain accurate clinical documentation.
- Coordinate HCA services and supervise care as required.
- Advocate for patient and family needs.
