Pride health is actively hiring a Registered Nurse to join our client's home health program in a growing care team serving patients.

Compensation:

• Travel Pay Package: Up to $2,526.00 weekly.

Location: Selinsgrove, PA, 17870

Schedule details (hours per day/week): Mon to Fri.

Productivity expectations:30 points/week

On Call/Weekend Requirement: No

Coverage Areas: Snyder/Union/Northumberland Counties.
Shift schedule (days, start & end times): Monday to Friday
Will Milage be reimbursed between visits, from first patient to last patient (Y/N)? Yes
 

Responsibilities:

• Complete skilled nursing visits, document in EMR, case confernece, interdisciplinary communication.
• Conduct comprehensive home health assessments, complete OASIS documentation, develop individualized Plans of Care (POC), and provide skilled nursing interventions to ensure quality patient care.
• Coordinate care with physicians, interdisciplinary teams, patients, and families while delivering education on medications, disease management, home safety, infection prevention, and self-care.
• Perform skilled nursing procedures including wound care, IV therapy, catheter and drain management, pain management, and other home health clinical interventions.
• Maintain accurate clinical documentation, supervise Home Health Aides (HHA/LVN/STNA), ensure compliance with regulatory and payer requirements, and participate in interdisciplinary meetings and on-call responsibilities as assigned.

Must have qualifications/experience: Home Health.
Type of certification required: RN licensure in PA or Comact License.
