What You'll Do

• Perform comprehensive physical therapy evaluations in patients' homes.
• Develop individualized treatment plans based on physician orders and patient goals.
• Treat patients to improve mobility, strength, balance, and functional independence.
• Complete OASIS assessments and maintain accurate, timely documentation in HCHB.
• Educate patients and caregivers on home exercise programs and safety strategies.
• Collaborate with physicians and the interdisciplinary care team to ensure optimal patient outcomes.
• Supervise Physical Therapist Assistants in accordance with state practice guidelines.

Assignment Highlights

✅ 40 hours guaranteed each week
✅ Monday–Friday | 8:00 AM – 5:00 PM
✅ Work only 2 weekend days per month
✅ No on-call responsibilities

Requirements

• Active Rhode Island Physical Therapist license or Compact PT License.
• Home Health experience required.
• HCHB experience required.
• OASIS experience required.
• Experience developing home exercise programs.
• Current BLS certification.
• Valid driver's license and reliable transportation.
