What You'll Do

• Perform comprehensive physical therapy evaluations in patients' homes.
• Develop individualized treatment plans based on physician referrals and patient goals.
• Help patients improve mobility, strength, balance, safety, and functional independence.
• Complete OASIS assessments and maintain accurate, timely documentation.
• Educate patients and caregivers on home exercise programs and fall prevention.
• Collaborate with physicians and the interdisciplinary healthcare team to ensure optimal patient outcomes.
• Supervise Physical Therapist Assistants in accordance with state practice guidelines.

Assignment Highlights

✅ Monday–Friday schedule | 8:00 AM – 5:00 PM
✅ No weekends or on-call responsibilities!
✅ Mileage reimbursement at $0.54 per mile

Requirements

• Active West Virginia Physical Therapist license or Compact PT License.
• Home Health experience required.
• OASIS experience required.
• HCHB experience preferred.
• Current BLS certification.
• Valid driver's license and reliable transportation.
