Job Description
We are seeking an experienced Acute Care Case Manager to support care coordination, discharge planning, and transition of patients across an acute care hospital setting. The ideal candidate will have strong hospital-based case management experience and be comfortable managing a high-volume caseload.

• Start Date: ASAP
• Setting: Acute Care Facility
• Experience: Minimum 2 years of acute medical Care Manager experience with strong care coordination skills
• Travel Experience: Required – First-time travelers not accepted
• License: Active California RN License required; pending license accepted if active by start date
• Floating: Must be willing to float to the Laguna facility

Key Responsibilities

• Coordinate care for acute medical patients and facilitate appropriate transitions of care.
• Collaborate with physicians, nurses, social workers, and other interdisciplinary team members.
• Assess patient needs and support effective discharge planning.
• Ensure continuity of care and appropriate utilization of healthcare resources.
