Acute Care RN Case Manager

Location: Apple Valley, CA 92307
Start Date: 08/31
Schedule: Days | 5x8s | 8:00 AM – 4:30 PM

💰 Compensation

Weekly Base Pay: $1,040
Weekly Non-Taxable Stipend: $1,470
Total Weekly Compensation: $2,510/week

📋 Requirements

2+ years of Acute Care Case Management / Discharge Planning experience
Must have acute hospital experience
Behavioral health experience alone does not qualify
California RN license required; pending license may be accepted if active by start date
Seasoned traveler required
Strong experience with:
- Patient assessments
- Care coordination
- Discharge planning
- Utilization review
- Admission criteria
- Prior authorizations
- DME coordination
- SNF placement
- Ambulance transportation
- Plan of care development
- Family meetings
- HMO/IPA processes
Must be able to independently arrange DME, SNF placement, and ambulance transportation

🏥 Required Hospital Experience

Experience working with patients across:

Emergency Department
ICU
Medical-Surgical
Surgical Services
