RN Care Coordinator (Peds) – Short Job Description

Department: Primary Care Clinic
Reports To: Manager, Primary Care Clinic

Position Summary

The RN Care Coordinator coordinates interdisciplinary care for pediatric patients, serving as a liaison between patients/families and healthcare providers. Responsibilities include coordinating appointments, referrals, records, travel arrangements, and treatment plans while educating families on healthcare needs, medications, resources, and self-care.

Key Responsibilities

  • Coordinate and monitor patient care plans and follow-up services.
  • Educate patients and families on conditions, medications, and self-care.
  • Coordinate specialty, outpatient, and external clinic appointments.
  • Process and track referrals through Cerner Referral Management.
  • Obtain and review medical records from outside facilities/providers.
  • Place provider-requested orders for labs, radiology, travel, and referrals.
  • Coordinate specialty clinics and healthcare-related events.
  • Communicate with providers, agencies, patients, and families to ensure timely care.
  • Provide relief RN coverage in the Outpatient Clinic as needed.
  • Maintain patient confidentiality and comply with organizational policies and standards.
  • Perform other duties as assigned.

Requirements

  • Associate’s or bachelor’s degree in nursing
  • Alaska RN License
  • BLS certification (AHA).
  • Strong nursing, communication, organization, and case-management skills.
  • Ability to manage a heavy caseload and work effectively with patients and families in emotional situations.
  • Experience with patient documentation and referral services preferred.
  • Knowledge of the pediatric patient population preferred.

Work Environment

  • Primarily office/clinic based.
  • Minimal travel required, including possible travel by aircraft, snowmobile, ATV, or boat.
  • Ability to perform routine clinical/office tasks and lift/carry up to 25 lbs.
RN Care Coordinator/Case Manager (Peds) - Shared Housing Provided Days - 8x5 | 13 weeks Sep 28th Star Date BLS (AHA) and PALS (AHA) required Alaska RN state license required. Recent experience required in Case Management/Care Coordinator.
