Job Title: Registered Nurse (RN) – Case Manager / Utilization Review

Location: Bozeman, Montana 59715
Shift: Day Shift | 7:30 AM – 4:30 PM
Contract: 13 Weeks
Schedule: Monday–Friday | Weekend Rotations

Mandatory Qualifications for this Job

Active Montana RN License
Current AHA BLS Certification
Minimum 2 years of nursing experience
Utilization Review (UR) experience is required
Strong case management and care coordination experience
Preferred: BSN
Preferred: 2 years of acute care hospital Case Management experience

Roles and Responsibilities

Assess patients and identify care management needs
Develop and implement individualized care plans
Perform Utilization Review and support appropriate levels of care
Monitor patient progress and care plan effectiveness
Identify and resolve barriers to timely care
Coordinate care with physicians and multidisciplinary teams
Support discharge planning and transitions of care
Coordinate home health, behavioral health, and community services
Document patient outcomes and interventions accurately
Manage assigned caseloads and conduct follow-ups
Promote continuity of care and ensure HIPAA compliance
