What You'll Do

• Perform comprehensive physical therapy evaluations in patients' homes.
• Develop individualized treatment plans based on physician orders and patient goals.
• Help patients improve mobility, strength, balance, safety, and functional independence.
• Provide education on home exercise programs, fall prevention, and safe mobility.
• Communicate patient progress and needs with physicians, caregivers, and the interdisciplinary team.
• Complete accurate and timely clinical documentation.

Requirements

• Active Oregon Physical Therapist license or Compact PT License.
• Home Health experience preferred.
• OASIS experience preferred.
• HCHB experience preferred.
• Current BLS certification.
• Valid driver's license and reliable transportation.
