Travel Physical Therapist (PT) – Home Health
Charlotte, North Carolina
We are seeking a Travel Physical Therapist for a 13-week Home Health contract in Charlotte, North Carolina. This opportunity offers the independence and flexibility of home health while providing meaningful one-on-one care to adult and geriatric patients in their homes.
This is a great fit for a PT who enjoys autonomy, patient-centered care, and helping individuals improve mobility, strength, safety, and overall independence in their home environment.

Responsibilities
- Perform comprehensive physical therapy evaluations and reassessments.
- Develop individualized treatment plans based on physician orders and patient goals.
- Treat orthopedic, neurological, post-surgical, cardiopulmonary, and medically complex patients.
- Provide gait training, balance training, therapeutic exercise, transfer training, and fall prevention education.
- Educate patients and caregivers on home exercise programs, safety strategies, and equipment use.
- Collaborate with nurses, physicians, case managers, and interdisciplinary care teams.
- Complete timely documentation and maintain home health compliance standards.
