Weekly Pay Package: $2,797/week
Home Health PT Opportunity in Greenville, NC
Ardor Health Solutions is seeking an experienced Physical Therapist for a 13-week Home Health assignment in Greenville, North Carolina.
This is a great opportunity for a PT who enjoys independence, flexibility, and providing individualized one-on-one care. You'll work directly with patients in their homes, helping them improve mobility, strength, balance, safety, and overall independence.

Qualifications Required
- Current North Carolina Physical Therapist license or eligibility to obtain prior to start
- Graduate of an accredited Physical Therapy program
- Current BLS certification
- Previous Home Health experience
- OASIS experience
- Homecare Homebase (HCHB) experience
- Ability to independently manage a home health caseload and daily territory
- Strong clinical assessment, documentation, and time-management skills
- Valid driver's license and reliable transportation
- Comfortable traveling throughout the assigned service area
- Ability to work independently while communicating effectively with the interdisciplinary care team
What You'll Do
- Complete PT evaluations and reassessments in patients' homes
- Develop individualized plans of care
- Provide gait, balance, strengthening, transfer, and mobility training
- Treat orthopedic, neurological, post-surgical, cardiopulmonary, and medically complex patients
- Provide fall-prevention and home-safety education
- Educate patients and caregivers on home exercise programs
- Coordinate care with nursing, OT, SLP, physicians, and other members of the healthcare team
- Complete timely and accurate documentation using Homecare Homebase (HCHB)
