🏠 Position Overview

Seeking an experienced Home Health Physical Therapist to provide evaluations, treatments, and patient education in the home setting. This role includes overseeing PTA plans of care and managing an independent caseload across multiple counties in Southern Illinois.

Hamilton County
Franklin County
Saline County

✅ Requirements

Active Illinois PT License
2+ years Physical Therapy experience
Home Health experience required
OASIS experience preferred
HCHB experience preferred
