Description

Pride health is actively hiring a Registered Nurse to join our client's home health program in a growing care team serving patients.

Compensation:

  • Local Pay: $50.00 (W-2)

    *(If your home location is within 50 miles from Charlotte, NC, 28216)

Location: Charlotte, NC, 28216

On Call/Weekend Requirement: on call 2-3 nights weekly with one weekend every 3-4 weeks

Coverage Areas: Surrounding areas.

Contract: 14+ Weeks
Hours: 40 Hours/Week
Shift: 8-Hour Shifts
Guaranteed Hours: 40 Hours/Week

Will Milage be reimbursed between visits, from first patient to last patient (Y/N)? Y ($0.38/Mile)

 

Responsibilities:

  • Conduct comprehensive in-home assessments, OASIS evaluations, and skilled nursing visits, developing and updating individualized Plans of Care based on patient needs.
  • Provide skilled clinical care including wound care, IV therapy, venipuncture, catheter/drain management, medication administration, pain management, and other home health interventions.
  • Coordinate care with patients, families, physicians, and interdisciplinary teams, providing education on medications, disease management, safety, infection prevention, and self-care.
  • Maintain accurate HCHB documentation, supervise HHA/LVN staff, participate in case conferences, monitor payer/regulatory compliance, and provide on-call support as assigned.

Required Qualifications:

  • Current Registered Nurse (RN) license in the applicable state.
  • 1–2 years of clinical RN experience required.
  • 2+ years of Home Health experience required.
  • Homecare Homebase (HCHB) experience – Required.
  • OASIS experience – Required.
  • Current CPR certification – Required.
  • Current driver’s license – Required.
  • Reliable transportation and valid auto insurance – Required.
  • Graduate of an accredited ADN or Nursing Diploma program – Required.
  • Strong knowledge of medications, skilled nursing procedures, patient assessment, and care coordination.
  • Ability to work independently, prioritize tasks, communicate effectively, and travel locally.
