• Role (RN, Tech, LPN, PT, etc.): RN Care Manager

• Specialty: Care Management

• Shift: 1st

• Weekend Hours: 36 (unless otherwise noted): occasional weekend hours available but not required

• Call Requirements: NO

• Years of Experience: 5 +

• Float Requirements: yes, would need to be able to cover Care Management department needs

• Patient Ratios: 1: 20-24

• Must Have Skills/Experience: discharge planning and care coordination and active RN license

• Personality Fit: flexible, easy going problem solver with calm demeanor and can do attitude
