<p> - M-F</p><p> - Oasis and Epic experience</p><p> - No overnight call. Call back required 2-4 times a month between the hours of 1700-2000. You will be told by 1700 on that day if an extra visit is needed. If by 1700 there is no visit scheduled, you do not stay on call.</p><p> - Case Manage 20-25 patient with an expectation of 5-6 visits per day; may use as re-vistor if agency needs change at any time during contract</p><p> - Adult med surg/oncology/Cardiac/Neurological/Respiratory/ Wound Care/GI/GU patient<br /> </p>
