Facility Name: Kindred Hospital Westminster

Trauma Level: n/a _ LTACH

Position Information:

• Role (RN, Tech, LPN, PT, etc.): PT
• Weekly Guaranteed Hours: 32-40 hrs per week
• Weekend Hours: just as needed
• Call Requirements: none
• Required Certifications: CA PT license, BLS
• Years of Experience: at least a year
• Float Requirements: 25 miles radius PRN

• Number of Beds: 113
• Patient Ratios: 8-10 patients per 8 hr shift

• Patient Population adult to geriatric

Hiring Profile:

• Must Have Skills/Experience: SNF/SAU or hospital setting
• Preferred Skillset/Experience: experience with trach/vent
• Common Diagnosis/Treatment: respiratory failure
• Personality Fit: open minded

• Required Education: graduate of PT with CA PT license
