Director of Rehabilitation Services

• Location: Montrose, NY 10548
• Full-time, onsite position
• Schedule: 40 hours/week, 7:00 AM–4:00 PM
• Rotating weekends and holidays
• Current New York State PT or OT license required
• Minimum 5 years of licensed therapist experience in an SNF rehabilitation department
• Previous rehabilitation leadership and staff supervision experience required
• Strong experience with quality assurance, clinical documentation, and regulatory compliance
• Knowledge of geriatric rehabilitation, long-term care, and resident safety
• Responsible for therapy department operations, staffing, scheduling, and program development
• Provide direct PT/OT services within NYS license
• Coordinate care plans, discharge planning, equipment needs, and interdisciplinary communication
• Support staff recruitment, orientation, competency validation, and in-service education
• Facility credentialing, physical exam, and TB/PPD documentation required before start
• Must maintain timely and accurate EHR documentation and resident confidentiality
