Job Title: Registered Nurse (RN) – Case Manager / Ambulatory Care

Location: Sitka, Alaska
Shift: Day Shift | 8:00 AM – 5:00 PM
Schedule: Monday–Friday
Contract: 13 Weeks

Mandatory Qualifications for this Job

Active, unrestricted Alaska RN License (Required at time of submission)
Graduate of an accredited School of Nursing
Bachelor of Science in Nursing (BSN) preferred
Current AHA BLS Certification
Case Management Certification preferred
Flu Vaccination Required
Fingerprint clearance required before start date
Strong ambulatory/outpatient nursing and care coordination experience

Roles and Responsibilities

Perform patient assessments and develop individualized care plans
Coordinate care with providers and multidisciplinary teams
Manage referrals, follow-up, and care transitions
Administer medications and treatments safely
Monitor patient progress and respond to changes
Educate patients and families on care plans
Maintain accurate EMR documentation
Promote continuity of care and patient advocacy
Ensure compliance with nursing and safety standards
Participate in quality improvement initiatives
Delegate clinical tasks appropriately
Float to other departments as needed

Candidates must have an active, unrestricted Alaska RN license at the time of submission. A current Flu Vaccination and fingerprint clearance are required before the start date, and onboarding will begin no earlier than six weeks after fingerprint cards are received. This is a 13-week, Monday–Friday day shift assignment in an ambulatory/outpatient setting, requiring strong case management, care coordination, patient assessment, and clinical documentation skills. A BSN and Case Management certification are preferred. Candidates should be comfortable working independently while collaborating with interdisciplinary teams to deliver high-quality, patient-centered care.
