Description

Job Title: Registered Nurse (RN) – Case Manager / Ambulatory Care

Location: Sitka, Alaska
Shift: Day Shift | 8:00 AM – 5:00 PM
Schedule: Monday–Friday
Contract: 13 Weeks
 

 Mandatory Qualifications for this Job

  • Active, unrestricted Alaska RN License (Required at time of submission)
  • Graduate of an accredited School of Nursing
  • Bachelor of Science in Nursing (BSN) preferred
  • Current AHA BLS Certification
  • Case Management Certification preferred
  • Flu Vaccination Required
  • Fingerprint clearance required before start date
  • Strong ambulatory/outpatient nursing and care coordination experience

 Roles and Responsibilities

  • Perform patient assessments and develop individualized care plans
  • Coordinate care with providers and multidisciplinary teams
  • Manage referrals, follow-up, and care transitions
  • Administer medications and treatments safely
  • Monitor patient progress and respond to changes
  • Educate patients and families on care plans
  • Maintain accurate EMR documentation
  • Promote continuity of care and patient advocacy
  • Ensure compliance with nursing and safety standards
  • Participate in quality improvement initiatives
  • Delegate clinical tasks appropriately
  • Float to other departments as needed

Candidates must have an active, unrestricted Alaska RN license at the time of submission. A current Flu Vaccination and fingerprint clearance are required before the start date, and onboarding will begin no earlier than six weeks after fingerprint cards are received. This is a 13-week, Monday–Friday day shift assignment in an ambulatory/outpatient setting, requiring strong case management, care coordination, patient assessment, and clinical documentation skills. A BSN and Case Management certification are preferred. Candidates should be comfortable working independently while collaborating with interdisciplinary teams to deliver high-quality, patient-centered care.
