Description
Job Title: Registered Nurse (RN) – Case Manager / Ambulatory Care
Location: Sitka, Alaska
Shift: Day Shift | 8:00 AM – 5:00 PM
Schedule: Monday–Friday
Contract: 13 Weeks
Mandatory Qualifications for this Job
- Active, unrestricted Alaska RN License (Required at time of submission)
- Graduate of an accredited School of Nursing
- Bachelor of Science in Nursing (BSN) preferred
- Current AHA BLS Certification
- Case Management Certification preferred
- Flu Vaccination Required
- Fingerprint clearance required before start date
- Strong ambulatory/outpatient nursing and care coordination experience
Roles and Responsibilities
- Perform patient assessments and develop individualized care plans
- Coordinate care with providers and multidisciplinary teams
- Manage referrals, follow-up, and care transitions
- Administer medications and treatments safely
- Monitor patient progress and respond to changes
- Educate patients and families on care plans
- Maintain accurate EMR documentation
- Promote continuity of care and patient advocacy
- Ensure compliance with nursing and safety standards
- Participate in quality improvement initiatives
- Delegate clinical tasks appropriately
- Float to other departments as needed
