Schedule: Monday–Friday, 8-hour shifts
Shift Options: 8:00 AM–4:30 PM or 8:30 AM–5:00 PM
Duration: 13 week with possible extension

Job Summary:
We are seeking a dedicated and experienced RN Case Manager to join our healthcare team. In this vital role, you will coordinate patient care across various settings, ensuring seamless transitions from hospital to post-acute care and outpatient services. Your expertise in clinical assessment, discharge planning, and utilization management will support optimal patient outcomes and enhance overall care quality.

Roles & Responsibilities:

• Develop and coordinate individualized discharge plans for patients.
• Provide care navigation and coordinate post-acute services.
• Educate patients and families regarding post-surgical care and discharge needs.
• Coordinate with providers, nursing staff, patients, families, and outside agencies.
• Support orthopedic care coordination and transition planning.
• Utilize EPIC, Parachute, and Careport as applicable to care coordination.
• Facilitate appropriate post-acute placement and services.
• Monitor patient needs throughout the hospitalization and assist with timely discharge.
• Collaborate with interdisciplinary teams to promote quality outcomes and continuity of care.
• Perform case management functions; utilization review is handled by another department.

Required Qualifications:

• Min 2 years of RN Case Manager experience required
• NY State license
• AHA -BLS
