Position Summary: Our Client is seeking a compassionate and organized RN Case Manager with strong communication and critical-thinking skills. The RN Case Manager will be responsible for coordinating patient care, assessing patient needs, developing care plans, and collaborating with healthcare providers to ensure appropriate and cost-effective care. The ideal candidate should be able to manage multiple patients while maintaining accurate documentation and providing quality patient-centered care.

Key Responsibilities:

• Assess patients and develop individualized care plans.
• Coordinate care with physicians, nurses, and other healthcare providers.
• Monitor patient progress and update care plans as needed.
• Coordinate admissions, discharges, referrals, and follow-up care.
• Educate patients and families about treatment and available resources.
• Assist with insurance authorization and utilization management when required.
• Maintain accurate patient records and documentation.
• Ensure continuity and quality of patient care.
