Home Health PT Opportunity – Albuquerque, NM
Ardor Health Solutions is seeking a Physical Therapist for a Home Health travel assignment in Albuquerque, New Mexico.
This is a great opportunity for a PT who enjoys autonomy, one-on-one patient care, and having more control over the flow of their day. You'll work primarily with adult and geriatric patients in their homes, helping them improve mobility, strength, balance, safety, and independence.

Qualifications Required
- Graduate of an accredited Physical Therapy program
- Current New Mexico PT license or ability to obtain prior to start
- Current BLS certification
- Previous Home Health experience preferred
- OASIS experience preferred
- Valid driver's license and reliable transportation
- Strong evaluation, documentation, and time-management skills
- Comfortable independently managing a Home Health caseload and daily territory
What You'll Do
- Complete PT evaluations and reassessments in patients' homes
- Develop individualized plans of care
- Provide gait, balance, strengthening, transfer, and mobility training
- Treat orthopedic, neurological, post-surgical, cardiopulmonary, and medically complex patients
- Provide fall-prevention and home-safety education
- Educate patients and caregivers on home exercise programs
- Collaborate with nursing, OT, SLP, physicians, and other members of the care team
- Complete timely clinical documentation
