Job Summary

Seeking an experienced Registered Nurse (RN) to provide skilled home health nursing care, coordinate individualized plans of care, and support patients and families in their homes. The RN will collaborate with physicians and the interdisciplinary team to ensure safe, quality, and compliant patient care.

Responsibilities

• Assess patients and develop, implement, and evaluate individualized Plans of Care.
• Provide skilled nursing care, including wound care, IV therapy, venipuncture, medications, catheter/drain care, and other clinical procedures.
• Educate patients and families on medications, disease management, safety, diet, and self-care.
• Monitor patient progress and maintain accurate clinical documentation.
• Supervise and provide clinical direction to LVNs/LPNs and home health aides.
• Coordinate care with physicians, patients, families, and interdisciplinary team members.
• Attend care conferences and interdisciplinary meetings as needed.
• Perform on-call duties and travel locally between patient homes.
• Follow infection control, OSHA, Medicare, and payer requirements.

Required Qualifications

• Current, unrestricted RN license.
• Graduate of an accredited ADN or Nursing Diploma program.
• 1–2 years of clinical RN experience and 2+ years of home health experience.
• Homecare Homebase (HCHB) experience required.
• OASIS experience required.
• Current CPR certification.
• Valid driver's license, reliable transportation, and auto insurance.
• Ability to work independently and manage a flexible schedule.

Preferred: BSN and CHPN certification.
