Travel Physical Therapist (PT) – Home Health

We are seeking a Travel Physical Therapist for a 16-week Home Health contract in White River Junction, Vermont. This opportunity offers the independence and flexibility of home health while providing meaningful one-on-one patient care in the home setting.

Responsibilities

• Perform comprehensive physical therapy evaluations and reassessments.
• Develop individualized treatment plans based on patient needs and physician orders.
• Treat orthopedic, neurological, post-surgical, cardiopulmonary, and medically complex patients.
• Provide gait training, balance training, therapeutic exercise, transfer training, and fall prevention education.
• Educate patients and caregivers on home exercise programs, safety strategies, and equipment use.
• Collaborate with nurses, physicians, case managers, and interdisciplinary care teams.
• Complete timely documentation and maintain home health compliance standards.
• Manage a home health caseload while self-scheduling visits based on patient needs.
