Mandatory Qualifications for This Job

Active, unencumbered Vermont and New Hampshire RN License OR Compact RN License
Graduate of an accredited nursing program
Minimum 2 years of Home Health RN experience
Strong case management and patient assessment experience

Roles and Responsibilities

Provide skilled home health nursing care in patients’ homes.
Conduct patient assessments and develop individualized care plans.
Coordinate care with physicians, caregivers, and interdisciplinary teams.
Monitor patient progress and report changes in condition.
Maintain accurate clinical documentation.
Educate patients and caregivers on medications, treatment, and self-care.
Coordinate services, case conferences, and safe patient discharges.
Deliver compassionate, patient-centered care.
