Job Title: Registered Nurse (RN) Home Health

Location: Rutland, Vermont 05701
Contract: 13 Weeks
Shift: Monday–Friday | 8:00 AM – 4:00 PM

Mandatory Qualifications for This Job

Active Vermont RN License or Compact RN License.
Minimum 2 years of Home Health RN experience preferred.
OASIS documentation experience is required.
Hospice coverage while on call is required.
Current BLS (AHA) certification.

Roles and Responsibilities

Assess patients and develop individualized care plans.
Provide skilled nursing care in patients' homes.
Manage patient caseloads and coordinate interdisciplinary care.
Complete accurate and timely OASIS documentation.
Administer medications, wound care, IV therapy, and treatments.
Educate patients and caregivers on care and disease management.
Collaborate with physicians and healthcare team members.
Monitor patient progress and update care plans.
Maintain accurate documentation and regulatory compliance.
Coordinate safe patient admissions, transfers, and discharges.

This is a 13-week Home Health RN contract in Rutland, VT, working Monday–Friday, 8:00 AM–4:00 PM. Candidates must have OASIS documentation experience, an active Vermont or Compact RN license, and AHA BLS certification. On-call, weekend rotation, holiday coverage, and hospice coverage while on call are required. Strong case management, clinical documentation, and home health nursing experience are highly preferred.
