Description

Job Title: Registered Nurse (RN) Home Health

Location: Rutland, Vermont 05701
Contract: 13 Weeks
Shift: Monday–Friday | 8:00 AM – 4:00 PM
 

 Mandatory Qualifications for This Job

  • Active Vermont RN License or Compact RN License.
  • Minimum 2 years of Home Health RN experience preferred.
  • OASIS documentation experience is required.
  • Hospice coverage while on call is required.
  • Current BLS (AHA) certification.

 Roles and Responsibilities

  • Assess patients and develop individualized care plans.
  • Provide skilled nursing care in patients' homes.
  • Manage patient caseloads and coordinate interdisciplinary care.
  • Complete accurate and timely OASIS documentation.
  • Administer medications, wound care, IV therapy, and treatments.
  • Educate patients and caregivers on care and disease management.
  • Collaborate with physicians and healthcare team members.
  • Monitor patient progress and update care plans.
  • Maintain accurate documentation and regulatory compliance.
  • Coordinate safe patient admissions, transfers, and discharges.

This is a 13-week Home Health RN contract in Rutland, VT, working Monday–Friday, 8:00 AM–4:00 PM. Candidates must have OASIS documentation experience, an active Vermont or Compact RN license, and AHA BLS certification. On-call, weekend rotation, holiday coverage, and hospice coverage while on call are required. Strong case management, clinical documentation, and home health nursing experience are highly preferred.
