Description

Pride Health is actively hiring a Physical Therapist (PT) to join our client's growing Home Health team.

 

Compensation:

Local Pay: $60.00 - $70.00/hour (W-2)

 

Will Milage be reimbursed between visits, from first patient to last patient (Y/N)? Yes (0.48/mile)

 

Schedule: Monday–Friday; 8:00 AM–4:30 PM
Productivity: 30 points/week
 

Key Responsibilities:

  • Conduct physical therapy evaluations and treatments in patients' homes.
  • Complete OASIS assessments and documentation.
  • Develop and implement individualized patient care plans.
  • Coordinate care with the interdisciplinary team.
  • Maintain accurate and timely clinical documentation.

Required Qualifications:

  • Active Physical Therapist (PT) License
  • HCHB experience required
  • BLS/CPR certification
  • Strong communication and time-management skills
