Description

Position: RN – Case Manager, Hospice

Contract: 13 weeks
Shift: Days
Hours: 9:30 AM–6:00 PM
Schedule: Flexible based on patient availability
Weekend: 1 weekend/month
Experience: 1+ year Hospice experience
License: Active CA RN license
Certification: BLS required
EMR: Homecare Homebase (HCHB)
Territory: Millbrae, Burlingame, San Mateo, Foster City, Belmont, Redwood City & nearby areas
Duties: Patient/family assessments, individualized care plans, follow-up visits, multidisciplinary care coordination & documentation
