We are seeking a Travel Occupational Therapist for a Home Health contract in White River Junction, Vermont. This opportunity offers the independence and flexibility of home health while providing meaningful one-on-one care to patients in their homes.

Responsibilities

• Perform comprehensive occupational therapy evaluations and reassessments.
• Develop individualized treatment plans based on patient needs and physician orders.
• Treat patients focused on ADLs, safety, mobility, functional independence, and home management.
• Provide education on adaptive equipment, home modifications, safety strategies, and caregiver support.
• Collaborate with nurses, physicians, case managers, and interdisciplinary care teams.
• Complete timely documentation and maintain home health compliance standards.
• Manage a home health caseload while coordinating visits based on patient needs.
