Home Health OT Opportunity – Pueblo, CO
Ardor Health Solutions is seeking an Occupational Therapist for a Home Health travel assignment with Interim HealthCare in Pueblo, Colorado.
This is a great opportunity for an OT who enjoys the independence and flexibility of Home Health while providing meaningful one-on-one patient care. You'll work primarily with adult and geriatric patients, helping them remain safe and independent in their homes following illness, injury, surgery, or hospitalization.

Qualifications Required
- Graduate of an accredited Occupational Therapy program
- Current Colorado OT license or ability to obtain prior to start
- Current BLS certification
- Previous Home Health experience preferred
- OASIS experience preferred
- Valid driver's license and reliable transportation
- Strong evaluation, documentation, and time-management skills
- Ability to independently manage a Home Health caseload and daily territory
What You'll Do
- Complete OT evaluations and reassessments in patients' homes
- Develop individualized plans of care
- Address ADLs, functional mobility, strength, endurance, cognition, coordination, and safety
- Perform home-safety assessments
- Provide fall-prevention education
- Recommend adaptive equipment and compensatory strategies
- Educate patients and caregivers
- Collaborate with PT, nursing, SLP, physicians, and other members of the Home Health team
- Complete timely clinical documentation
