SNF OT Opportunity – Montesano, WA
Ardor Health Solutions is seeking an Occupational Therapist for a Skilled Nursing Facility travel assignment in Montesano, Washington.
This is a great opportunity for an OT who enjoys working with an adult and geriatric rehab population. You'll help patients regain independence with everyday activities following hospitalization, surgery, illness, or injury, with treatment focused on improving function, safety, and quality of life.

- ADL and functional independence-focused treatment
- Collaborative interdisciplinary rehab team
Qualifications Required
- Graduate of an accredited Occupational Therapy program
- Current Washington OT license or ability to obtain prior to start
- Current BLS certification
- Strong evaluation and treatment skills
- Strong clinical documentation and communication skills
- Comfortable working with adult and geriatric patients
- SNF or geriatric rehab experience preferred
What You'll Do
- Complete OT evaluations and reassessments
- Develop individualized plans of care
- Address ADLs, strength, endurance, functional mobility, cognition, coordination, and safety
- Provide therapeutic activities and functional training
- Recommend adaptive equipment and compensatory strategies
- Educate patients and caregivers
- Collaborate with PT, PTA, SLP, nursing, and the interdisciplinary team
- Assist with discharge planning
- Complete timely clinical documentation
