Home Health PT Opportunity – Dover, DE

Ardor Health Solutions is seeking an experienced Physical Therapist for a 13-week Home Health assignment in Dover, Delaware, providing coverage throughout Kent County.

This is a Monday–Friday position with no weekends and no on-call requirements. You'll provide one-on-one physical therapy in patients' homes while independently managing your daily schedule and territory.

Qualifications Required

  • Graduate of an accredited Physical Therapy program
  • Current Delaware PT license or Compact privilege
  • Current BLS certification
  • Previous Home Health experience required
  • Homecare Homebase (HCHB) experience required
  • OASIS experience required
  • Valid driver's license and reliable transportation
  • Strong evaluation, documentation, and time-management skills
  • Ability to independently manage a Home Health caseload and territory

What You'll Do

  • Complete PT evaluations and reassessments in patients' homes
  • Complete applicable OASIS documentation
  • Develop individualized plans of care
  • Provide gait, balance, strengthening, transfer, and mobility training
  • Treat orthopedic, neurological, post-surgical, and medically complex patients
  • Provide fall-prevention and home-safety education
  • Educate patients and caregivers on home exercise programs
  • Collaborate with nursing, OT, SLP, physicians, and other members of the Home Health team
  • Complete clinical documentation using HCHB
  • Maintain a minimum productivity expectation of 30 points/week

This position covers Kent County, Delaware. Weekly mileage varies depending on patient census and daily routing. Exact coverage expectations and typical mileage can be discussed during the interview.
