Home Health PTA Opportunity – Fort Mill, SC
Ardor Health Solutions is seeking a Physical Therapist Assistant for a Home Health travel assignment with CenterWell in Fort Mill, South Carolina.
This is a great opportunity for a PTA who enjoys the independence of Home Health and providing one-on-one care to adult and geriatric patients in their homes. You'll work under the direction of a Physical Therapist to help patients improve mobility, strength, balance, safety, and functional independence.

Qualifications Required
- Graduate of an accredited Physical Therapist Assistant program
- Current South Carolina PTA license or ability to obtain prior to start
- Current BLS certification
- Minimum 1 year of Home Health experience
- Homecare Homebase (HCHB) experience
- Valid driver's license and reliable transportation
- Strong documentation and time-management skills
- Ability to independently manage a daily Home Health route
What You'll Do
- Provide treatments under the direction of a Physical Therapist
- Carry out established plans of care in patients' homes
- Provide gait, balance, strengthening, transfer, and mobility training
- Help patients improve safety and functional independence
- Reinforce home exercise programs
- Educate patients and caregivers
- Monitor progress and communicate changes to the supervising PT
- Collaborate with nursing, OT, SLP, and other members of the Home Health team
- Complete timely documentation using HCHB
