Location: San Mateo, California
Duration: 13 Weeks (Possible Extension)
Shift: Days | 9:30 AM – 6:00 PM | 40 Hours/Week | 1 Weekend/Month
Travel Pay: Gross Weekly Up to $2,600/week
EMR: Homecare Homebase (HCHB)
Mileage Reimbursement: $0.76/mile

Learn Beyond Healthcare LLC is seeking an experienced Registered Nurse (RN) Case Manager – Hospice for a 13-week contract assignment in San Mateo, California. The Hospice RN will admit patients to hospice services, assess patient and family needs, develop individualized plans of care, coordinate with multidisciplinary teams, and provide follow-up care while supporting quality hospice services.

 

Territory: Millbrae, Burlingame, San Mateo, Foster City, Belmont, Redwood City, and neighboring areas as needed.

 

Requirements:

  • Active California RN License – Required
  • BLS Certification – Required
  • Minimum 1 year of recent Hospice case management experience – Required
  • Territory: Millbrae, Burlingame, San Mateo, Foster City, Belmont, Redwood City, and neighboring areas as needed.
  • Must be available for 1 weekend per month – Required
  • Holiday coverage may be required
