Description
Care Coordinator
Schedule: 8-hour shifts, 8:00 AM–4:30 PM, weekdays and weekends
Position Summary
The Care Coordinator manages and coordinates patient care throughout the episode of illness or treatment in collaboration with patients, families, physicians, care teams, and payors. The role focuses on clinical care coordination, utilization management, resource optimization, and advocacy to ensure safe, efficient, and cost-effective outcomes.
Key Responsibilities
- Assess, plan, coordinate, and evaluate patient care.
- Coordinate services with patients, families, physicians, multidisciplinary teams, and payors.
- Review admissions, discharges, observation stays, readmissions, Medicare criteria, and utilization cases.
- Monitor patient progress and resource utilization.
- Support Emergency Department care coordination when assigned.
- Initiate appropriate clinical pathways based on admission diagnosis.
- Advocate for patients and families throughout the care episode.
- Facilitate communication among providers and monitor services provided.
- Coordinate appropriate discharge, transfer, or alternate levels of care.
Minimum Requirements
- Minimum 1 year of recent Case Management experience in an acute care setting.
- Current California RN License.
- Current CPR certification.
