Registered Nurse (RN) – Home Health
Job Type: Contract
Location: Local Home Health Visits
Schedule: 8-hour shifts | 40 hours/week guaranteed
Travel: Local travel between patient homes required
Job Summary
We are seeking an experienced Home Health Registered Nurse (RN) to provide skilled nursing care to patients in their homes. The RN will perform comprehensive assessments, develop and manage individualized Plans of Care, coordinate with interdisciplinary teams, and educate patients and families to promote safe and effective care at home.
Key Responsibilities
- Conduct comprehensive home health assessments in stable and emergent situations.
- Develop, implement, document, and evaluate individualized Plans of Care (POC).
- Complete accurate OASIS assessments and documentation.
- Provide skilled nursing care, including medication administration, wound care, IV care, venipuncture, catheter/drain care, and pain management.
- Educate patients and families regarding medications, diet, disease management, home safety, and self-care.
- Monitor patient progress and communicate changes to physicians and the interdisciplinary care team.
- Coordinate referrals and services based on patient needs.
- Supervise and provide clinical direction to LVNs/LPNs and HHAs.
- Maintain accurate and timely clinical documentation.
- Participate in interdisciplinary team meetings and case conferences.
- Perform on-call responsibilities as assigned.
- Ensure compliance with clinical, regulatory, and third-party payer requirements.
- Maintain patient confidentiality and follow infection-control and OSHA standards.
Required Qualifications
- Current Registered Nurse (RN) license
- 2+ years of Home Health experience
- 1–2 years of clinical RN experience
- HCHB (Homecare Homebase) experience – Required
- OASIS experience – Required
- Current CPR/BLS certification
- Valid driver's license
- Reliable transportation and required auto insurance
- Graduate of an accredited ADN or Nursing Diploma program
