Description
Physical Therapist – Home Health
Location -Brunswick, GA
Shift: 8 hours/day
Guaranteed: 40 hrs/week
Contract: 14 weeks
Job Summary
We are seeking a licensed Physical Therapist to provide home-based physical therapy services to patients of all ages. The PT will evaluate patients, develop individualized Plans of Care, provide treatments, educate patients and families, and collaborate with physicians and interdisciplinary teams.
Key Responsibilities
- Perform patient evaluations and develop/update Plans of Care.
- Provide therapy for neurological, neuromuscular, and orthopedic conditions.
- Teach safe transfers, mobility techniques, and use of assistive devices.
- Recommend and manage appropriate durable medical equipment.
- Educate patients and families on treatment and home exercise programs.
- Maintain accurate clinical documentation and follow infection-control standards.
- Collaborate with physicians and other healthcare professionals.
Requirements
- Current state Physical Therapist license – Required.
- Current CPR certification – Required.
- Valid driver's license, reliable transportation, and insurance – Required.
- Bachelor's degree in Physical Therapy from an APTA-approved program – Required.
- 1–2 years of acute care or rehabilitation experience – Required.
- Home health experience – Preferred.
Ability to travel locally and perform physical activities including lifting, bending, pushing, pulling, standing, and walking.
