Description

RN – Case Manager | Hospice |

Location: Sunnyvale, CA (Bay Area, CA)
Duration: 13 weeks
Shift: Days | Sunday–Thursday
Schedule Options: 10:00 AM–7:00 PM or 11:00 AM–8:00 PM
Schedule: Flexible based on patient availability
Hours: 40 hrs/week

Territory:

San Francisco County
San Mateo County
Santa Clara County

Requirements:

Minimum 1 year of Hospice experience
Active state CA RN license
BLS/BCLS required
Hospice RN experience
Home health/hospice field experience preferred
EMR: Homecare Homebase (HCHB)
Bilingual: Not required
Holiday coverage required

Responsibilities:

Admit patients to hospice services
Assess physical, psychosocial, environmental and safety needs
Develop individualized plans of care
Document patient assessments and interventions
Coordinate care with multidisciplinary teams
Conduct follow-up visits as needed
Participate in quality assessment and performance improvement programs
