Pharmacist (AHCL)

Position Overview

The Pharmacist is responsible for providing safe, effective, and compliant medication management services, collaborating with physicians, nurses, and other healthcare professionals to ensure optimal patient care.

Key Responsibilities

• Accurately review and document medication protocols and therapy orders in the medical record, including TPN, pharmacokinetic dosing, and heparin protocols.
• Communicate with physicians and nursing staff regarding medication orders, discrepancies, errors, questions, or irregularities and intervene when appropriate.
• Monitor medication therapy through the computerized patient record system to promote medication safety and efficacy, including monitoring medication errors and conducting nursing unit inspections.
• Monitor and document controlled substances in accordance with applicable laws and regulations.
• Evaluate medication regimens for drug-drug interactions, allergies, therapeutic/toxic drug levels, appropriate indications, and overall medication safety.
• Provide clinical drug information and medication guidance to physicians, nurses, and other hospital staff through direct consultation and committee participation.
• Monitor pharmacy inventory and remove expired, outdated, or compromised medications to maintain medication quality and safety.
• Supervise pharmacy support personnel in routine activities, including IV admixture preparation, medication dispensing, and medication compounding.
• Maintain all required licenses, certifications, competencies, and education in accordance with Adventist Health, The Joint Commission, CMS, and other applicable regulatory requirements.
• Perform additional pharmacy duties and responsibilities as assigned.

Schedule

• Shift: Day
• Schedule: 4x10s — 4 days per week
• Hours: 6:30 AM–5:30 PM
• Days: No set days; weekend coverage may be required occasionally
• Experience: 1–2 years preferred

Required Qualifications

• Graduate of an accredited College of Pharmacy.
• Current and active California Pharmacist License.
• Epic experience required.
• Pyxis experience required.
• Hospital pharmacy experience preferred.
• Pharmacy residency training preferred.
• Ability to communicate and collaborate professionally with hospital staff, patients, and the public.
• Strong judgment, integrity, professionalism, self-awareness, and emotional maturity.
• Self-motivated and detail-oriented with the ability to maintain confidentiality and handle sensitive information appropriately.
• Current knowledge of HIPAA and applicable confidentiality requirements.

Physical & Work Environment Requirements

• Frequent bending, squatting, kneeling, climbing, reaching above the shoulders, walking, standing, sitting, and repetitive hand movements.
• Ability to talk, hear, and perform duties requiring close vision and adjustment of focus.
• Ability to occasionally lift boxes and supplies weighing up to 25 pounds, including lifting a 25-pound box from the floor.
• Potential Category 3 exposure to blood and body fluids as defined by the facility.
• Ability to use general office and pharmacy-related equipment, including computers, printers, calculators, copy machines, and cell phones.
