Description

Pride Health is actively hiring a Registered Nurse (RN) to join our client's growing Home Health team.

Location: Fayetteville, NC 28304; Coverage Area: Robeson, Hoke & Cumberland Counties

Local Pay: $53.00 to $58.00/hour (W-2)

Mileage Reimbursement: $0.49/mile (Mileage is not reimbursed between patient visits (first patient to last patient.)

Weekly Hour Guarantee: 40 hours

Schedule: Monday–Friday: 8:00 AM–4:30 PM

Productivity: 30 points/week

On-Call/Weekend: Yes

Job Responsibilities:

Provide skilled nursing care in patients’ homes, including assessments, medications, treatments, wound care, and monitoring changes in condition.
Develop and update individualized plans of care while coordinating with physicians, therapists, patients, families, and interdisciplinary teams.
Complete accurate and timely OASIS and clinical documentation in the EMR while maintaining compliance with home health regulations.
Educate patients/caregivers on disease management, medications, safety, and home care procedures while ensuring quality, patient-centered care.

Required Qualifications:

Active RN License required.
Previous nursing experience required.
Ability to provide independent skilled nursing care in a home-based setting.
Valid driver's license and reliable transportation.

Preferred Qualifications:

Home Health experience strongly preferred.
HCHB (Homecare Homebase) experience preferred.
OASIS documentation experience is highly desirable.
Strong assessment, communication, documentation, and care-coordination skills.
