<h3>Inpatient Pharmacist</h3>Position: Pharmacist – Inpatient Shift: Nights – 9:00 PM–7:00 AM Schedule: 7 days on / 7 days off Contract: 13 weeks Weekend: Rotating as needed Holiday: Required Call: None EMR: Cerner Dispensing System: Omnicell <h3>Responsibilities</h3><ul><li>Provide inpatient pharmacy services across ICU, IMC, ED, Med-Surg, OR, cardiac cath, EPS, central pharmacy, and IV room.</li><li>Perform medication dosing and monitoring.</li><li>Manage Vancomycin AUC dosing.</li><li>Perform aminoglycoside dosing.</li><li>Manage and monitor heparin therapy.</li><li>Manage and monitor TPN.</li><li>Support medication preparation and verification.</li><li>Understand USP 797 requirements and ensure appropriate IV medication preparation.</li><li>Collaborate with physicians, nurses, pharmacy technicians, and other healthcare professionals.</li><li>Ensure safe and accurate medication distribution.</li></ul><h3>Requirements</h3><ul><li>Pharmacist license – Wisconsin</li><li>BLS (AHA) required</li><li>Inpatient staffing experience required</li><li>Vancomycin AUC dosing experience</li><li>Aminoglycoside dosing experience</li><li>Heparin dosing/monitoring experience</li><li>TPN management/monitoring experience</li><li>Understanding of USP 797</li><li>BCPS preferred</li><li>IV room experience is not required.</li></ul>
