Schedule

• Day shift
• 12-hour shifts
• 36 hours per week
• Rotating weekends and holidays
• No on-call requirement
• Self-scheduling may be possible

Required Qualifications

• Valid Pharmacist license in Wisconsin
• 1 year of Pharmacist experience
• Pharmacist experience within the last year
• BLS
• Recent acute hospital experience
• Automated Dispensing Cabinets experience
• Experience with Epic

Key Responsibilities

• Review medication orders for appropriateness, including dose, dosage form, regimen, therapeutic duplication, and drug interactions
• Provide pharmacokinetic and anticoagulation dosing
• Support pharmaceutical care through medication reconciliation and discharge medication education
• Oversee pharmaceutical compounding compliance with USP 797 and USP 800 guidelines

Work Setting

• Critical access hospital
• Clinical coverage may include emergency, intensive care, medical/surgical, behavioral health, infusion, and surgical areas
