• Weekend Hours: Every other weekend availability, Holidays
• Call Requirements: NO
• Years of Experience: 5 + Hospital case management, 5+ Hospital acute adult care nursing
• Float Requirements: yes, you would need to be able to cover Care Management department needs

• Number of Beds: 32
• Patient Ratios: 1: 20-24
• Equipment:
• EMR: EPIC, Wellsky, Careport/PASSR/Teams, Laptop, Phone (ascom)
Hiring Profile:
• Must Have Skills/Experience: discharge planning and care coordination, case management minimum 5 years
• Preferred Skillset/Experience: More than 5+ years nursing practice in hospital
• Type of Patients on Unit: High acuity, complex, high percentage behavioral health
• Personality Fit: flexible, easy going problem solver with calm demeanor and can do attitude
