JOB OVERVIEW

Seeking an experienced Pharmacist to provide safe and accurate medication management in an acute care pharmacy setting. The Pharmacist will review medication orders, support clinical teams, monitor drug therapy, and promote medication safety.

ROLES & RESPONSIBILITIES

• Review medication orders for appropriate dosing, allergies, and contraindications.
• Ensure safe and accurate medication use.
• Perform pharmacokinetic monitoring and consultations.
• Provide medication recommendations to healthcare providers.
• Report medication-related adverse events and variances.
• Provide medication counseling to patients and families.
• Support pharmacy education and student training.
• Follow hospital pharmacy policies and procedures.

REQUIREMENTS

• Minimum 1 year pharmacy experience as a Pharmacist or Pharmacy Intern
• Hospital Pharmacy experience preferred
• BLS Certification required
• CADD pump programming experience
• Basic Pyxis experience
• Medication safety reporting experience
• EPIC KBMA barcode scanning experience
• Familiarity with electronic drug information resources such as UpToDate and NeoFax
• Ability to access and follow pharmacy policies and procedures
• Strong medication safety and clinical judgment skills
