Inpatient Case Manager – Discharge Planning

Department: Case Management

We are seeking an experienced Inpatient Case Manager to support discharge planning for patients on Med-Surg and Telemetry units. This role requires a strong understanding of inpatient discharge processes, community resources, post-acute referrals, and interdisciplinary collaboration.

Qualifications

  • Minimum 1 year of inpatient case management experience required
  • BLS certification required
  • PRI & SCREEN certifications required; certifications may be completed online
  • Critical care experience is preferred
  • Familiarity with CarePort, MCG, and/or InterQual preferred
  • Previous Utilization Review experience is a plus
  • Strong critical-thinking, organizational, communication, and time-management skills
  • Ability to work independently while collaborating effectively with physicians, social workers, nurses, patients, and other members of the care team
  • Must be comfortable managing a caseload of at least 20 patients

Position Responsibilities

The Case Manager will focus primarily on inpatient discharge planning rather than Utilization Review. Responsibilities include:

  • Completing comprehensive discharge planning for hospitalized patients
  • Participating in daily interdisciplinary rounds with physicians and the care team
  • Coordinating discharges to home, community settings, SNFs, and other appropriate destinations
  • Identifying barriers to discharge and developing appropriate solutions
  • Making referrals to post-acute and community-based services
  • Assisting with housing resources and shelter arrangements for patients experiencing homelessness
  • Coordinating specialized discharge needs, including hemodialysis-related documentation and wound VAC/KCI referrals
  • Maintaining effective communication with patients, families, providers, and community facilities
  • Using available departmental resources and reference materials to support complex discharge planning

Note: Utilization Review is handled by a separate department, which is responsible for insurance authorization and transportation arrangements. Physicians are responsible for medication reconciliation and sending prescriptions/orders to the pharmacy.

Caseload & Unit Coverage

Most units have approximately 33–40 patients, generally divided between one Case Manager and one Social Worker. The Case Manager typically carries 17–25 patients.

On designated pilot units, one Case Manager may work alongside two Social Workers, resulting in a typical caseload of 12–17 patients.

Case Managers are generally assigned to Med-Surg or Telemetry units following Internal Medicine and Family Medicine services.

Schedule

  • Monday–Friday
  • 8:00 AM–4:30 PM
  • 7.5-hour workday with a 1-hour lunch
  • Full lunch breaks are supported by management
  • Weekend and holiday coverage is uncommon and generally requires manager approval
  • Weekend/holiday coverage may require supporting multiple units

Orientation

New hires will complete approximately 2 days of hospital orientation, including Allscripts/EMR training, followed by approximately 1–2 weeks of unit-based preceptorship, depending on competency and progress.

Dress Code

Business casual or scrubs are permitted. Scrubs may be any color except Royal Blue or Maroon, with neutral colors preferred. A lab coat is provided.
