Home Health PT Opportunity – Flint, MI

Ardor Health Solutions is seeking an experienced Physical Therapist for a 13-week Home Health travel assignment with Humana/CenterWell in Flint, Michigan.

This position is ideal for a PT who enjoys the independence of Home Health and providing meaningful one-on-one care to adult and geriatric patients. You'll manage your own daily territory while helping patients regain mobility, strength, balance, safety, and independence in their homes.

Qualifications Required

  • Graduate of an accredited Physical Therapy program
  • Current Michigan PT license or ability to obtain prior to start
  • Current BLS certification
  • Minimum 1 year of Home Health experience
  • Homecare Homebase (HCHB) experience
  • OASIS experience
  • Valid driver's license and reliable transportation
  • Strong evaluation, documentation, and time-management skills
  • Ability to independently manage a Home Health caseload and daily territory

What You'll Do

  • Complete PT evaluations and reassessments in patients' homes
  • Complete applicable OASIS assessments and documentation
  • Develop individualized plans of care
  • Provide gait, balance, strengthening, transfer, and mobility training
  • Treat orthopedic, neurological, post-surgical, cardiopulmonary, and medically complex patients
  • Provide fall-prevention and home-safety education
  • Educate patients and caregivers on home exercise programs
  • Collaborate with nursing, OT, SLP, physicians, and other members of the Home Health team
  • Complete timely clinical documentation using HCHB
