Travel Physical Therapist (PT) – Outpatient Rehab

We are seeking a Travel Physical Therapist for a 13-week outpatient rehab contract in Monument, Colorado. This opportunity offers the chance to work with patients focused on improving mobility, strength, function, and overall quality of life in a collaborative clinic-based setting.

Responsibilities

• Perform comprehensive physical therapy evaluations and reassessments.
• Develop individualized treatment plans based on patient goals and functional needs.
• Treat orthopedic, post-surgical, sports medicine, neurological, and general musculoskeletal conditions.
• Provide therapeutic exercise, manual therapy, gait training, balance training, and functional mobility interventions.
• Educate patients on home exercise programs, injury prevention, and safe movement strategies.
• Complete timely documentation and progress reporting.
• Collaborate with the clinic team to support strong patient outcomes.
