Pay Breakdown:

• Weekly Gross: $2,950
• W2:  $18/hr.
• Stipend:  $2,275 Weekly

Job Summary:
The RN Case Manager coordinates comprehensive patient care by assessing needs, developing care plans, facilitating discharge planning, and coordinating services across the healthcare team. The role focuses on improving patient outcomes, ensuring appropriate resource utilization, and supporting safe transitions of care.

Key Responsibilities

• Assess patient needs and develop individualized care plans.
• Coordinate care with physicians, nurses, social workers, and other healthcare professionals.
• Manage discharge planning and transitions of care.
• Monitor patient progress and ensure appropriate services are provided.
• Facilitate referrals, authorizations, and community resources.
• Educate patients and families regarding treatment plans and available resources.
• Review medical records and support appropriate utilization of services.
• Maintain accurate case management documentation.
• Advocate for patients and address barriers to care.
• Ensure compliance with facility policies, regulatory requirements, and standards of care.

Requirements

• NY State RN license - Required
• Minimum 2 years of recent Case Management experience - Required
• Must have experience with discharge planning, referrals and some utilization review
• Current BLS AHA certifications - Required
• PRI & SCREEN certifications  - Must be willing to take after offer (Amount will be reimbursed in 1st or 2nd paycheck)
