Description
RN – Case Manager | Eureka, CA
Location: Eureka, CA
Start: 09/21/2026
Duration: 13 weeks
Shift: Days | 5×8s | 8:00 AM–4:30 PM
Requirements:
• 2+ years acute care hospital Case Management experience
• Case Management experience must be current within the last year
• Care coordination & discharge planning experience required
• Experience with HMOs, IPAs, advance directives, POLST & protective services
• Epic experience required
• Active CA RN License
• Travel experience preferred; first-time travelers may be considered with approval
Additional:
• Patient caseload up to 25
• Patient ratio approximately 1:20
• Every other weekend required
• No call
• May float based on department needs
• Locals accepted
• Free parking on first day
