Avg 14 patients a day case load

All inpatient
DC to home health, rehab, SNF, LTAC, LTC
Adult population

Skills required:
Experience with prior authorization for post-acute care
DME planning
Working with and communication with a diverse interdisciplinary team
Time management and multi-tasking
Support within the Department:
Shared office for CM team
Each unit staffs one CM and one SW
Receptionist
Charge nurse
Unit manager
