Travel Physical Therapist (PT) – Home Health
Crestview Hills, Kentucky
Client: Amedisys Home Health
Setting: Home Health
Schedule: Monday–Friday, 8:00 AM–5:00 PM
Contract: 13 Weeks
Territory: Boone, Kenton, Campbell & Pendleton Counties
Productivity: 30 points/week
EMR: Homecare Homebase (HCHB)
Weekends/On-Call: None
Home Health PT Opportunity – Crestview Hills, KY
Ardor Health Solutions is seeking an experienced Home Health Physical Therapist for a 13-week contract with Amedisys in Crestview Hills, Kentucky.
This is a Monday–Friday, 8 AM–5 PM position with no weekends and no on-call requirements. The PT will provide home-based therapy throughout Boone, Kenton, Campbell, and Pendleton Counties in Northern Kentucky.

What You'll Do
The PT will evaluate and treat patients in their homes, develop individualized plans of care, complete reassessments, provide gait, balance, strengthening and mobility interventions, educate patients and caregivers, and coordinate care with the interdisciplinary Home Health team.
The therapist should be comfortable independently managing a mobile caseload, completing timely documentation, and organizing the workday to meet the 30-point weekly productivity expectation.
Requirements
- Graduate of an accredited Physical Therapy program
- Active Kentucky PT license or ability to obtain prior to start
- Current BLS
- Previous Home Health experience is required
- Homecare Homebase experience preferred
- Comfortable traveling throughout the assigned territory
- Strong independent case-management and documentation skills
